Commonwealth Home Support Programme (CHSP)
CHSP is an entry-level home support program that helps elderly to live independently in their homes and communities.
It focus on helping senior with their everyday tasks, doing it with them rather than doing it for them.
There are varieties of support program under CHSP, GentleCare is specialised in the following categories:
- Allied Health and Therapy Services (Occupational Therapy, Physio Therapy, Dietician, Remedial massage. etc)
- Nursing (Contingency & Risk Assessment, Wound Care, Medication. etc)
- Other Food Services (Food Preparation)
- Social Support Group (Community group event)
Once you have been approved for the Commonwealth Home Support Program, My Age Care Team will send you a referral code, one code per approved category. With this code, Sunny Care can immediately organise services for you.

Commonwealth Home Care Packages (HCP)
There are 4 levels of Home Care Packages from Level 1 basic care need to Level 4 high care need, this support is designed for elderly with more complex care needs that go beyond what the Commonwealth Home Support Programme (CHSP) can provide.
In other work, Under HCP, GentleCare can do tasks for you.
The Home Care Package is an integration of the entry-level CHSP.
Within the package, care recipient can coordinate mix of services includes: help with household tasks, purchasing large equipment, minor home modification, personal care, clinical care and Allied health services.
