Support At Home (SAH) Program Overview

From 01/11/2025, all home care package (HCP) clients are transited to be SAH clients.

Support at Home is a new approach to aged care that allows you to access a tailored mix of services depending on your health, daily living needs, and personal goals. Funded by the Australian Government through My Aged Care, funding ranges from basic support to intensive care — so your services can grow and adapt as your needs change.

What Home Support Can We Provide for You?

Support at Home covers a broad range of services designed to meet your physical, emotional, and everyday living needs:

Clinical Care

Includes nursing care, wound management, allied health (like physiotherapy or podiatry), nutrition support, and care management to coordinate your services.

Independence

Assistance with personal care (such as bathing and dressing), help with mobility, social support and companionship, in-home therapies, and respite care for carers.

Everyday Living

Support with day-to-day tasks like cleaning, laundry, meal preparation, home maintenance, and transport to appointments or activities.

 

Services We Provide

Allied Health Services

 This comprises a range of specialist services such as physiotherapy, podiatry, occupational therapy, speech therapy, Remedial Massage .etc 

Personal Care

 Assistance with your daily routine hygiene care at home such as grooming, showering, toileting .etc 

Domestic Assistance

 Including general household tasks such as cleaning, laundry, meal preparation, we can also do grocery for you 

Transport

 Accompany you with doctor’s appointments, shopping, banking, outings, and other outdoor activities. 

Plan Manager

Our dedicated plan manager will handle the financial administration of your NDIS plan.

Gardening and Yard Maintenance

 We work with reliable gardeners and handyman who will take care of your garden and trees. 

Nursing care

Professional nursing support for your health and medical needs at home.

Social outings

We accompany you to appointments, shopping, and community activities.

Medical equipment

Access to assistive devices and medical equipment for daily living.

Home modification

Safe, accessible home changes tailored to your mobility needs.

How to Access Your Home Care Services

  • Contact us for a free consultation to check your eligibility.

  • Contact My Aged Care to request an assessment (takes 4–12 weeks).

  • Complete the aged care assessment to determine your care level.

  • Receive your funding approval (usually takes 6–8 months).

  • Contact us again to start your personalised home care services.
